The Case Management Team's goal is to assist service users to attain and maintain long-term housing by providing wrap around support services which includes one-to-one outreach support, twenty-four hour on-call availability, regular follow-up, and flexing work schedules to better meet the needs of our service users.
The Case Management Team serves females, males or transgendered persons with or without children who are accessing services through WEAC, Elizabeth House, Crossroads Program, and/or E4C's continuum of housing.

- Support
- Referrals to community agencies
- Assistance with housing search, identification, clothing, food, transportation, and financial security
- Life skills coaching including money management/budgeting, shopping, cooking, hygiene, and coaching on socially acceptable behaviours
- Legal needs including referrals and court support
- Help with housing set up including furniture, household goods, damage deposits, assistance in accessing payment of utilities, and utility hook ups
- Housing maintenance through regular home visits
- Liaising with landlords to ensure housing stability
- Support for medical needs
- Assist with addictions and relapse prevention
- Emotional support including referrals to counseling for anger management, self-esteem, and personal growth to learn new ways to cope
- Support and referral for healthy pregnancy and childbirth
- Advocating with Children and Family Services
- Assist with repatriation with children
- Parenting skills and childcare
- Job search
- Referrals for education programs and upgrading
- Connecting service users with resources within their community
